Please read these Terms of Service carefully before using NoteForWork.com. By accessing or using our service, you agree to be bound by these terms.
NoteForWork.com provides asynchronous telehealth services for the limited purpose of physician-signed work and school absence documentation for minor illnesses. The clinical services are provided by Manavjeet Sidhu, MD, A Professional Corporation, a California professional medical corporation.
The Service is available in select states only. You are required to select your state of physical location at the time of your service request. The state you select determines whether the Service is available to you and governs the applicable standard of care for your encounter.
We may use your IP address and other available signals as part of our compliance monitoring. Your affirmative location attestation is the controlling representation of your physical location.
If your state is not currently available, you will be offered the opportunity to provide your email address to be notified when the Service becomes available in your state.
The fee for each physician-reviewed note request is $29.00 USD. Payment is processed securely through Stripe. You will NOT be charged if your request is declined by the physician for clinical reasons or if a technical failure prevents delivery.
If your note is not delivered within 60 minutes of physician approval due to a platform error on our part, you are entitled to a full refund. Contact ceo@noteforwork.com within 7 days. Refunds are NOT available if your request was declined for clinical reasons, you provided inaccurate information, your employer declined the note, or you changed your mind after delivery.
We guarantee delivery of your physician-signed note within 60 minutes of physician approval. If delivery is delayed beyond 60 minutes due to reasons within our control, you are entitled to a full refund. Guarantee applies to delivery time following physician approval only.
You agree NOT to provide false information, obtain documentation for conditions you do not have, use this service during a medical emergency, or use notes in a fraudulent manner.
NoteForWork.com enforces the following documentation limits per user account:
(a) Maximum of two (2) consecutive calendar days of absence documentation per request; and
(b) Maximum of four (4) total days of absence documentation in any rolling thirty (30) day period.
These limits are enforced at the platform level and are designed to ensure that our Service is used for its intended purpose: documenting minor, short-duration illnesses that do not require in-person medical evaluation. Users who reach these limits within the applicable period will receive a recommendation to seek in-person care.
PROHIBITED CONDUCT. The following conduct is expressly prohibited and constitutes a material breach of these Terms:
(i) Submitting requests under different names, nicknames, name variations, or aliases (including but not limited to the use of a legal name, nickname, middle name, or abbreviated name) for the purpose of exceeding the documentation limits stated above;
(ii) Providing false, misleading, or inaccurate information in the intake questionnaire;
(iii) Submitting a false or inaccurate state of location (see Section 3);
(iv) Sharing account credentials with another individual for the purpose of obtaining additional documentation;
(v) Using the Service for any purpose other than documenting a genuine, minor illness experienced by the registered account holder.
CONSEQUENCES OF MISUSE. NoteForWork.com reserves the right to: (a) immediately suspend or permanently terminate any account found to have engaged in prohibited conduct; (b) void and retract any absence note issued based on fraudulent or misleading information; (c) report confirmed fraud to the user's employer if NoteForWork.com receives an inquiry from that employer; and (d) pursue available legal remedies.
VERIFICATION. Employers and third parties may verify the authenticity and usage status of any note issued by NoteForWork.com by visiting noteforwork.com/verify/[NOTE-ID] or contacting verify@noteforwork.com. NoteForWork.com cooperates fully with legitimate employer verification inquiries.
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, NOTEFORWORK.COM AND ITS PHYSICIANS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF THIS SERVICE, INCLUDING BUT NOT LIMITED TO EMPLOYMENT CONSEQUENCES OR DENIAL OF DOCUMENTATION BY YOUR EMPLOYER.
Our total liability shall not exceed the amount you paid for the specific request giving rise to the claim.
NoteForWork.com bears no liability for services rendered in reliance on false or inaccurate information submitted by the user, including a false state of location.
We cannot guarantee that any employer, school, or institution will accept the documentation provided. Employer policies vary and are outside our control.
All clinical decisions are made exclusively by the reviewing physician in the exercise of independent medical judgment. NoteForWork.com does not direct or influence the physician's clinical determinations.
Your use of this service is subject to our Notice of Privacy Practices and Privacy Policy, both incorporated by reference. Your health information is protected under HIPAA.
These Terms are governed by the laws of the State of California. Disputes shall be resolved by binding arbitration in Fresno County, California under AAA rules, except either party may seek injunctive relief in court.
