Please read these Terms of Service carefully before using NoteForWork.com. By accessing or using our service, you agree to be bound by these terms.

1. Description of Service

NoteForWork.com provides asynchronous telehealth services for the limited purpose of physician-signed work and school absence documentation for minor illnesses. The clinical services are provided by Manavjeet Sidhu, MD, A Professional Corporation, a California professional medical corporation.

2. Eligibility

• Be 18 years of age or older, or the parent/legal guardian of the patient.
• Be physically located in a state in which our physicians are licensed at the time of your intake submission.
• Have symptoms consistent with a minor, self-limiting illness.
• Not be experiencing a medical emergency.

3. Payment Terms

The fee for each physician-reviewed note request is $29.00 USD. Payment is processed securely through Stripe. You will NOT be charged if your request is declined by the physician for clinical reasons or if a technical failure prevents delivery.

4. Refund Policy

If your note is not delivered within 60 minutes of physician approval due to a platform error on our part, you are entitled to a full refund. Contact ceo@noteforwork.com within 7 days. Refunds are NOT available if your request was declined for clinical reasons, you provided inaccurate information, your employer declined the note, or you changed your mind after delivery.

5. 60-Minute Delivery Guarantee

We guarantee delivery of your physician-signed note within 60 minutes of physician approval. If delivery is delayed beyond 60 minutes due to reasons within our control, you are entitled to a full refund. Guarantee applies to delivery time following physician approval only.

6. Acceptable Use

You agree NOT to provide false information, obtain documentation for conditions you do not have, use this service during a medical emergency, or use notes in a fraudulent manner.

7. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, NOTEFORWORK.COM AND ITS PHYSICIANS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF THIS SERVICE, INCLUDING BUT NOT LIMITED TO EMPLOYMENT CONSEQUENCES OR DENIAL OF DOCUMENTATION BY YOUR EMPLOYER.

Our total liability shall not exceed the amount you paid for the specific request giving rise to the claim.

8. No Guarantee of Employer Acceptance

We cannot guarantee that any employer, school, or institution will accept the documentation provided. Employer policies vary and are outside our control.

9. Physician Independence

All clinical decisions are made exclusively by the reviewing physician in the exercise of independent medical judgment. NoteForWork.com does not direct or influence the physician's clinical determinations.

10. Privacy

Your use of this service is subject to our Notice of Privacy Practices and Privacy Policy, both incorporated by reference. Your health information is protected under HIPAA.

11. Governing Law and Dispute Resolution

These Terms are governed by the laws of the State of California. Disputes shall be resolved by binding arbitration in Fresno County, California under AAA rules, except either party may seek injunctive relief in court.
