Copy and email your results
After completing all cases, copy your results summary and email it to ceo@noteforwork.com. No CV or additional documents are needed at this stage.

Medical director review
Dr. Manavjeet Sidhu, MD reviews all applications personally. Qualified candidates are contacted within 5 business days for a brief follow-up call.

Credentialing and onboarding
Selected candidates complete the credentialing checklist, sign the Independent Contractor Agreement and Business Associate Agreement (BAA), and receive platform access prior to launch.

About this assessment

This assessment presents 7 patient cases. The first is a practice case and does not count toward your results.

● LOW

These patients are excellent candidates for a work excuse note. They present with extremely low clinical and medicolegal risk — no red flags, no risk factors, and no concerning features. Our protocols recommend to approve.

△ BORDERLINE

These patients have passed the automated screener but present with comorbid conditions or symptoms that warrant physician judgment — such as pregnancy with low-grade fever. There is no right or wrong answer. Both approve and decline are clinically defensible. Use your own judgment.

Availability rate for logged-in scheduled windows, plus $10 per note reviewed.

Compensation structure is discussed individually with each selected candidate. Final arrangement is documented in the Independent Contractor Agreement prior to onboarding.

Requirements
Active unrestricted MD, DO, or NP license in at least one target state
Clean disciplinary history — no active investigations or restrictions
Professional liability insurance — minimum $1M/$3M coverage
Not excluded from Medicare, Medicaid, or any federal healthcare program
Board certification (MD/DO) or NP certification in relevant specialty
NPs must hold full practice authority in their state to sign independently
Employed by a facility with a competing telehealth conflict of interest
Active felony charges or healthcare-related criminal history
